Job summary

Seamlessassist is hiring a Remote Care Navigator for Cardiac care coordination. The role focuses on telephonic outreach, care plan support, and CMS-compliant documentation for medically complex CHF patients. You will work under RN supervision, escalating clinical concerns appropriately.

The position is full-time, 40 hours per week, remote within the United States with CST/PST overlap required. Prior MA or LVN/LPN certification is required, plus at least 2 years in care coordination and strong

Qualifications

• Active MA certification or LVN/LPN license.
• Minimum 2 years in care coordination, case management, or ambulatory care.
• Familiarity with CMS PCM, CCM, and/or TCM documentation standards.
• Proficient with care coordination software and EHRs.
• AI fluency; actively uses AI tools to work faster and more efficiently.
• Based in the United States and authorized to work; US business hours overlap.
• Excellent written and verbal English communication; strong phone presence.

Responsibilities

• Conduct structured telephonic outreach to CHF and complex cardiac patients.
• Maintain an assigned patient caseload using risk stratification to prioritize outreach.
• Complete initial assessments and follow-ups on symptoms, meds, psychosocial status and SDOH.
• Support CMS-compliant documentation and care plan updates.
• Coordinate with RN Care Managers, cardiologists, and PCP offices.
• Provide patient education on CHF self-management strategies.
• Document time, interventions, and patient goals per CMS standards.
