Alignment Health is seeking a remote Care Coordinator to support its Special Needs Program (SNP) case management team. This role helps members with complex health needs by collaborating with the RN Case Manager and interdisciplinary care team.

You will conduct outreach, coordinate referrals and prior authorizations, monitor hospital/SNF discharges, and maintain accurate member records while ensuring confidentiality in a fast-paced remote environment.

Qualifications

• Bilingual in Spanish or Vietnamese is required.
• Minimum 1 year of experience in a healthcare IPA setting.
• Minimum 1 year of experience with referrals and prior authorizations.
• High school diploma or GED is required; Bachelor’s degree preferred or 4 years of experience in lieu of degree.
• Basic computer skills: Outlook, Word, Excel; typing 25+ WPM.

This position plays a critical role in assisting members with complex health needs by working closely with the RN Case Manager and interdisciplinary care team.
