Job summary

UnitedHealth Group is seeking a Wellness Coordinator to provide non-clinical care coordination and administrative support for behavioral health and substance abuse needs. You will engage members, screen for care gaps, and develop individualized plans to overcome access barriers, while supporting the Behavioral Health Case Management team with referrals and chart summaries.

Responsibilities include patient outreach, documentation, and collaboration with clinical partners.

Qualifications

  • 2+ years of managed care or call center experience
  • 1+ years of computer proficiency
  • 1+ years of experience handling sensitive issues with members and providers
  • 1+ years of dual diagnosis experience with mental health and substance abuse
  • 1+ years of experience in medical/behavioral health care coordination
  • Ability to work 40 hours weekly in specified time zones
  • Access to a quiet home workspace with PHI security

Responsibilities

  • Handle inbound and outbound member queue
  • Document in electronic systems
  • Coordinate information to clinical team partners
  • Provide behavioral health benefit education
  • Screen for gaps in care and contact members
  • Provide network referrals and community resources
  • Provide administrative support
  • Perform other duties as assigned
