Job summary

A mental health services organization in New York is seeking a full-time Counselor to provide supportive counseling and assist residents with daily activities. The ideal candidate should have a minimum of a High School Diploma and at least 1 year of experience in a residential or mental health setting. Qualifications

Responsibilities

  • Provide counseling and behavioral interventions to residents.
  • Assist residents in daily activities and community engagement.
  • Monitor medications and provide crisis management.
  • Assist residents in developing service plan goals and supports.
  • Conduct fire safety training and ensure drills meet regulatory guidelines.
  • Document resident interactions in line with agency policies.
  • Attend weekly meetings with primary residents to review goals.
  • Maintain contacts with outside providers and participate in case conferences.
  • Support residents in obtaining and maintaining entitlements.
  • Monitor medications as required.
  • Provide visits to residents in apartments and 24-hour supervision as needed.
  • Provide transportation using personal vehicle when agency vehicle is unavailable.
  • Report emergencies and safety issues to supervisors.
  • Participate in team meetings and meet minimum shift requirements.
  • Complete program trainings and QA-related activities.
  • Share data to support informed decisions with clients.

JOB DEFINITION

Provides supportive counseling, ADL training, crisis intervention and restorative services (when applicable) to people with psychiatric disabilities in residential programs in compliance with relevant regulations.

EDUCATIONAL REQUIREMENTS

Minimum of a High School Diploma, Bachelor's Degree Preferred.

EXPERIENCE REQUIRED

Minimum of 1-year experience in a residential or mental health setting. If currently employed by CN Guidance, Minimum of 1-year of employment in CN Guidance Residential department in good standing.

DUTIES AND RESPONSIBILITIES
  1. Provide counseling, behavioral interventions, restorative services, assistance in negotiating community activities and crisis management services.
  2. Monitor, train and assist residents in all programs in household maintenance activities to ensure a clean, safe and homelike environment. Directly participate in household maintenance activities when required.
  3. Provide training, supervision and assistance to residents in daily activities including food purchases, recreational activities, and meal planning and personal care activities.
  4. Assist residents in developing meaningful service plan goals and provide supports to assist residents in achieving their goals
  5. Conduct fire safety training activities and ensures fire drills are conducted in accordance with regulatory guidelines.
  6. Participate in all Medicaid billable activities under leadership of the Supervisor, and/or ensure that all documentation required for the particular program is current and in compliance with regulatory guidelines.
  7. Document interactions with residents in accordance with agency policies and standards.
  8. Meet weekly, or as scheduled, with assigned primary residents for counseling and to establish and review service plan goals.
  9. Maintain collateral contacts with outside service providers and participate in case conferences as scheduled.
  10. Assist residents in procuring and maintaining financial entitlements.
  11. Monitor medications as required.
  12. Make visits to residents in apartments as scheduled and/or provide 24-hour supervision to residents in congregate facilities.
  13. Provide transportation as needed using personal vehicle when agency vehicle is not available.
  14. Promptly report all emergencies, incidents, maintenance and safety issues to senior Counselor or Supervisor.
  15. Participate in team meetings as scheduled.
  16. Work a minimum of 1 shift monthly within the department.
  17. Complete program trainings as assigned.
  18. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress
  19. Complete QI Training during onboarding and participate in QI refresher training as needed.
