EMPACT-SPC is seeking a Rehabilitation Specialist to assess and implement services per the individual service plan. You will document progress toward recovery and coordinate related rehabilitation services with RSA counselors and providers.

The role emphasizes coordinating activities for consumers with serious mental illness and facilitating their involvement in meaningful community employment or activities.

Qualifications

• Must have a minimum of a high school diploma/GED and 4 years of full-time behavioral health work experience.
• Two years of work experience in behavioral health, preferably with adults diagnosed with serious mental illness.
• Adequate computer and typing skills; experience with Outlook, Excel, Word, and Next Gen preferred.
• Bi-lingual (Spanish) preferred.

What you’ll do:

As a member of the clinical team, the Rehabilitation Specialist completes assessment and service plan and implements services to consumers in accordance with their individualized service plan (ISP). Communicates and documents consumers’ progress toward their recovery. Coordinates and monitors related services for consumers with serious mental illness.

This job is responsible for coordinating and monitoring rehabilitation-related services for consumers living with a serious mental illness. Under the Vocational and Rehabilitation Services Interagency Governmental Agreement (IGA), works in partnership with assigned Rehabilitation Services Administration (RSA) Vocational Rehabilitation Counselor(s), community rehabilitation program providers, and others involved in assisting consumers to become involved in or maintain involvement in meaningful community activities or employment.

What you’ll provide:

• Must have a minimum of High school diploma/GED and 4 years of full time Behavioral Health work experience or meet BHT requirements.
• Must have Two years of work experience in behavioral health, preferably providing vocational/employment and/or rehabilitation services to adults diagnosed with a serious mental illness in an interdisciplinary clinical team environment.
• Experience working with various community Agencies/Providers in employment, VR, education, self-help, peer support, and other areas is preferred.
• Must have an understanding and application of best practices in psychiatric rehabilitation.
• Adequate computer and typing skills are required.
• Experience working with Outlook, Excel, Microsoft Word, and Next Gen preferred.
• Must have a valid AZ driver's license with no major infractions in the past three years.
• If driving while on agency time or on agency business, a valid Arizona driver license and appropriate liability insurance and approval for liability coverage with EMPACT - SPC’s liability insurance carrier is required. At time of hire, a 3-year state driving record with no major infractions or excessive driving violations/tickets is required.
• Must have regular access to reliable transportation.
• Bi-lingual (Spanish) preferred. Language Diff per hour available.
• Must be at least 21 years of age at the time of employment.
