Job summary

Cardinal Hill Rehabilitation Hospital, part of Encompass Health, seeks a Rehabilitation Registered Nurse to provide comprehensive, post-acute care for patients recovering from stroke, brain and spinal cord injury, and complex medical conditions.

The RN will complete assessments, develop and update individualized care plans, administer medications and treatments, and coordinate with an interdisciplinary team to maximize function, safety, and independence.

Responsibilities

• Perform comprehensive assessments of patients to determine rehabilitation needs.
• Develop and update individualized care plans in collaboration with the interdisciplinary team.
• Administer medications and treatments as prescribed.
• Coordinate care with physicians, therapists, and other caregivers to maximize function and safety.
• Provide patient and family education to support recovery and self-management.
• Assist with discharge planning and community reintegration strategies.
• Accurate EMR documentation of patient status and progress.
