Personal Touch Home Health Care Services Inc. in Apex, NC, is seeking a Registered Nurse (Rehabilitation) to provide compassionate inpatient and outpatient rehab nursing care to adults recovering from injury, surgery, stroke, or serious illness.

You will conduct comprehensive patient assessments, administer medications and treatments, monitor progress, and collaborate with an interdisciplinary team to develop individualized rehab plans.

Registered Nurse (Rehabilitation)

Provide compassionate inpatient and outpatient rehabilitation nursing care to adults recovering from injury, surgery, stroke, or serious illness. Conduct comprehensive patient assessments, administer medications and treatments, and monitor patient progress to support safe recovery.

Work closely with an interdisciplinary team to help develop and implement individualized rehab plans. Educate patients and families on care needs and recovery goals, and promote independence and quality of life.

Maintain strict safety, quality, and clinical standards, including supporting safe transitions of care.
