Job summary

English Oaks Nursing and Rehabilitation Center, a Generations Healthcare facility, in Modesto, CA, is seeking a Director of Rehabilitation (PT or OT) to lead our skilled nursing and rehabilitation team. The role focuses on high-quality patient care, coordinating a large therapy unit, and discharging patients home or to lower levels of care.

Qualifications

• Active CA Physical Therapist or Occupational Therapist license required.
• Minimum 1 year of clinical SNF experience.
• Ability to practice unencumbered and in compliance with regulatory standards.
• Proficient in English with strong communication and teamwork.

Responsibilities

• Directly supervises and guides all therapy personnel within the facility.
• Manage daily department scheduling to meet facility needs.
• Provide direct patient care and determine appropriate caseload with management.
• Ensure rehabilitation needs of residents are met and documented accurately.
• Review therapy services and update programs to reflect changing needs.
• Coordinate denials management and ensure timely claims and appeals.
