• Assessment - evaluate data across psychological, physiological, sociocultural, spiritual, economic and lifestyle factors.
• Plan – set measurable goals based on the assessment and standards of care.
• Implement – coordinate plan of care, education and discharge planning.
• Evaluate – re-evaluate progress and revise care as needed.
