• Monitor and assess patient conditions, identifying potential issues and implementing interventions as necessary
• Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans
• Communicate effectively with patients, families, and interdisciplinary team members to ensure seamless transitions and optimal care
• Maintain accurate and detailed records of patient care, including progress notes and treatment plans
• Participate in quality improvement initiatives and contribute to the development of policies and procedures
