Job summary

Bethany Home, Inc. in Waupaca, WI seeks a compassionate Rehabilitation Registered Nurse (Rehab RN) to help patients regain function and independence. You will assess needs, develop care plans, administer treatments, and monitor progress in a multidisciplinary team.

You’ll educate patients and families, coordinate discharge planning, and stay current with best practices in rehabilitation nursing while delivering high-quality, patient-centered care.

Qualifications

• Must hold a valid RN license in the state of practice.
• BSN degree preferred; CRRN certification preferred or willingness to obtain.

Responsibilities

• Conduct comprehensive patient assessments to determine rehabilitation needs.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient progress and adjust care plans to achieve rehabilitation goals.
• Educate patients and families about treatment plans and self-care techniques.
• Collaborate with therapists and other rehabilitation professionals to optimize outcomes.
• Document patient care activities and maintain up-to-date medical records.
• Ensure compliance with health and safety regulations and protocols.
• Assist in discharge planning to ensure continuity of care post-rehabilitation.
• Stay updated on best practices and advancements in rehabilitation nursing.
