UofL Health - Frazier Rehab Hospital Brownsboro in Louisville, KY, seeks a Licensed Practical Nurse (LPN) for a PRN role to support nursing operations under RN supervision. The position focuses on direct patient care, medication administration, assessments, and coordination with the care team to optimize patient outcomes.

The LPN will participate in implementing rehabilitation nursing interventions, updating care plans, and educating patients and families.

Formación

• LPN license required.
• Basic Life Support certification required within 30 days of hire.
• CPR/BLS certification required; IV certification highly preferred or must be obtained within 6 months.
• Minimum of six months of Medical/Surgical experience in an acute care setting preferred.

Responsabilidades

• Administer medications safely, accurately, and based on patient age and clinical condition.
• Minimize potential for medication errors by following proper medication administration procedures.
• Assist the RN with initial assessments and patient reassessments per policy.
• Identify and implement appropriate rehabilitation nursing interventions.
• Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
• Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
• Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
• Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
• Develop and implement individualized teaching plans based on patient and family learning needs.
• Perform other duties as assigned.
