Job summary

LiveWell Homecare Agency, LLC in St. Louis, MO is seeking an RN to provide direct patient care in a 9-bed, open-unit with 24-hour coverage. You will collaborate with PT, OT, and Speech therapists to support recovery and functional restoration.

You will develop, implement, and document personalized care plans, assess physical, psychosocial, and emotional needs, and educate patients and families on illness management and home care, with a strong focus on safety and regulatory compliance.

Qualifications

• Nursing Diploma or Associate’s Degree and RN license required.
• Bachelor’s degree preferred with at least 2 years of experience.

Role Overview:

Responsibilities include:

• Promoting patient and family-centered care, including education on illness management, post-treatment home care, and medication administration.
• Participating in activities that ensure patient safety, quality improvement, and regulatory compliance.
• Developing, implementing, and documenting personalized care plans with clear goals, ensuring effective communication across care teams and with patients and families.
• Assessing and evaluating physical, psychosocial, and emotional needs, considering patient preferences, values, and cultural factors.
• Utilizing critical nursing skills to provide comprehensive care in a safety-sensitive environment.

Qualifications:

• Minimum: Nursing Diploma/Associate’s Degree and RN license.
• Preferred: Bachelor’s Degree in Nursing and at least 2 years of experience.
