Job summary

Orlando Health Bayfront Hospital is seeking a Clinical Liaison to coordinate referrals and pre-admission assessments for an acute inpatient rehab facility in St. Petersburg, FL.

The role requires building relationships with physicians, payers, and referral sources, and facilitating timely patient placement with the Medical Director. Responsibilities include on-site patient evaluations, coordinating admission decisions, and supporting marketing efforts while ensuring CMS compliance.

Qualifications

• Degree from an accredited school and licensed in a health-care related field.
• Minimum of two (2) years of clinical experience with demonstrated skills in clinical assessment and marketing.
• Minimum of two (2) years experience in acute care or rehab preferred.
• Requires professional level of knowledge in physician relations and interfacing with insurance companies.
• Extensive knowledge of local provider and medical communities.
• Good communication skills.
• Ability to work harmoniously with other personnel.
• Must be able to follow written and oral instructions.
• Maintains a current license as Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Respiratory Therapy, or Licensed Clinical Social Worker in State of Florida.
• Must maintain a valid Florida driver’s license.

Responsibilities

• Assist and coordinate the referral and pre-admission process with appropriate documentation of patient assessment in accordance with CMS regulations.
• Conduct on-site clinical evaluation of patients, including chart review and interviews with physicians, nurses, and therapists.
• Demonstrate knowledge of growth, development and aging for age-appropriate care planning.
• Secure information relating to patients’ resources and benefits; obtain insurance authorization for admission.
• Provide information to patients and families about the facility and program; arrange tours as needed.
• Notify referral sources of admission decisions and transfer times.
• Coordinate transitions from referring facility or home to the rehab facility.
• Establish contacts with attending physicians and care team to ensure timely admissions.
• Complete educational in-services with physicians, case managers and other professionals.
• Contribute to market analyses and support marketing plans for the rehab facility.
• Maintain reports related to referrals, admissions, denials and census.
• Maintain regular attendance per policy; ensure compliance with policies and procedures.
• Serve as a resource to staff and healthcare professionals; participate in community awareness events.
• Be available for occasional evenings, weekend coverage, and day travel.
