Job summary

Life Care Centers of America is seeking an Occupational Therapist to provide direct rehab services in SNF settings, focusing on long-term care and short-term rehab. You will collaborate with an interdisciplinary team to help patients achieve functional progress and improve quality of life in a patient-centered environment.

Required NBCOT certification, OT degree (BSOT/MSOT), active state license, CPR on hire, and welcoming new grads.

Responsibilities

• Follow occupational therapy treatment plans for patients (ADLs)
• Establish, assess, and modify goals that are realistic and functional
• Oversee and evaluate care by OTAs and Rehab Aides
• Chart properly and timely
• Use therapy software accurately
• Provide excellent customer service and positive patient attitude
• Assist in patient evacuation when needed
• Maintain dependable attendance and professional conduct
• Collaborate within an interdisciplinary team
• Adhere to policies, confidentiality, and compliance

Setting & Population Served

Life Care Centers of America facilities operate as Skilled Nursing Facilities (SNFs) that provide:

• Long term care for residents who require ongoing skilled support and compassionate daily assistance
• Short term, sub-acute rehabilitation for patients recovering from surgery, illness, or injury
• Collaborative care through an interdisciplinary team approach with nursing, therapy, and medical staff working together
• A diverse patient population including individuals with orthopedic, neurological, cardiopulmonary, and post-acute needs

Position Summary

The OT - Occupational Therapist provides direct rehab care in occupational therapy to patients upon physician referral in accordance with all applicable laws, regulations, and Life Care standards.

Education, Experience, and Licensure Requirements

• NBCOT certified upon hire, but renewal is optional going forward
• Graduate of an accredited program in occupational therapy (BSOT or MSOT)
• Currently registered/licensed in applicable State. Must maintain an active license in good standing throughout employment.
• One (1) year experience preferred; new grads welcome to apply
• CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.

Specific Job Requirements

• Participates in community health matters/projects as appropriate
• Proficient in Microsoft Word, Excel, and e mail
• Liaisons with patients, families, support departments, etc., to adequately plan for patient needs
• Must demonstrate good body mechanics at all times
• Make independent decisions when circumstances warrant such action
• Knowledgeable of practices and procedures as well as the laws, regulations, and guidelines governing functions in the post acute care facility
• Implement and interpret the programs, goals, objectives, policies, and procedures of the department
• Perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation
• Maintains professional working relationships with all associates, vendors, etc.
• Maintains confidentiality of all proprietary and/or confidential information
• Understand and follow company policies including harassment and compliance procedures
• Displays integrity and professionalism by adhering to Life Care'sCode of Conductand completes mandatoryCode of Conductand other appropriate compliance training

Essential Functions
