Job summary

The Life Care Center of Paradise Valley AZ is seeking an Occupational Therapist to provide direct rehab care to patients upon physician referral, in accordance with Life Care standards. Requirements include NBCOT certification upon hire, a BSOT or MSOT degree, and current state licensure with CPR certification on hire.

You will collaborate with OTAs and rehab staff, document care, and support patient goals in a team-driven setting.

Qualifications

• NBCOT certification upon hire, renewal optional.
• Graduate of an accredited OT program (BSOT or MSOT).
• Currently registered/licensed in the applicable State; active license required.
• One year experience preferred; new grads welcome to apply.

Responsibilities

• Follow occupational treatment plans for patients and goals.
• Assess and modify goals to be measurable and functional.
• Oversee and evaluate care by OTAs, Rehab Aides, and students.
• Chart accurately and timely; use therapy software as required.
• Provide excellent customer service and work on an interdisciplinary team.
• Maintain confidentiality and adhere to policies and safety.

Position Summary

The OT - Occupational Therapist provides direct rehab care in occupational therapy to patients upon physician referral in accordance with all applicable laws, regulations, and Life Care standards.

Education, Experience, and Licensure Requirements
