The Outpatient PACT Registered Nurse organizes team members to achieve desirable patient outcomes through day‑to‑day case management in a Primary Care Clinic using a Patient Aligned Care Team model. Education Debt Reduction Program (Student Loan Repayment): APPROVED!

Qualifications

• English Language Proficiency – Basic written and spoken English, in accordance with 38 U.S.C. 7403(f).
• Graduate of a school of professional nursing approved by the appropriate accrediting agency (ACEN or CCNE) or completing a master's level bridge program with coursework equivalent to a bachelor's level degree in Nursing. Upon state license attainment may be appointed temporarily and later permanently after graduation.
• Foreign school graduates must possess a current, full, active, and unrestricted registration. Credit for foreign nursing education greater than associate/entry level requires formal degree equivalency validation from a recognized equivalency evaluation (e.g., ICD).
• Current, full, active, unrestricted registration as a graduate professional nurse in a U.S. state, territory, Commonwealth or the District of Columbia.
• Graduate Nurse Technician (GNT) Exception: Candidates meeting basic education but lacking required licensure may be appointed at the entry step for a 120‑day temporary appointment while pursuing licensure, extendable to two years on a case‑by‑case basis.
• Grade Determinations per scope and dimension criteria.
  • Level I (Nurse I) – requires an ADN or Diploma in Nursing.
  • Level II (Nurse I) – requires an ADN or Diploma with 1 year of specialized nursing experience or a BSN with no additional experience.
  • Level III (Nurse I) – requires an ADN or Diploma with 2 years of experience or a BSN with 1 year of experience, or an MSN/BSN/related field with 1 year or an MSN with 0 experience.
  • Level I (Nurse II) – requires a BSN with 2 years of experience or a MSN with 1 year of specialized experience.
  • Level I (Nurse III) – requires a MSN with 2 years of specialized experience, a related master’s with 2 years, or a Doctoral in Nursing or related field with appropriate experience.
• Preferred Experience: 1–2 years Ambulatory Care, 1–2 years Med‑Surg, and triage experience (telephone and/or face‑to‑face). Ambulatory Care Certification is an asset.
• Physical Requirements: Straight pulling (6 h), pushing (2 h), walking (6 h), standing (2 h), repeated bending (6 h). Visual requirement (20/30 corrected).

Duties

• Demonstrates clinical knowledge in assessing, planning, implementing, documenting, and evaluating care for designated patients across the continuum.
• Triage and apply a collaborative team approach to identify, analyze, and resolve patient care problems.
• Collaborate with patients to assess and identify needs, issues, care goals, and resources to achieve desired outcomes for discharge, post‑hospital recovery, and wellness.
• Maintain current knowledge of multidisciplinary resources and programs, referring patients to community services as appropriate.
• Utilize problem‑solving methodologies to advance clinical knowledge and improve systems or processes impacting patient care.
• Assist navigation of health systems, intervening or referring to facilitate patient outcomes.
• Provide care respecting patient’s personal values, cultural, and belief systems.
• Assist with development, assessment, and monitoring/evaluation of comprehensive care plans to meet efficiency, effectiveness, and coordination standards.
• Coordinate an interdisciplinary approach to ensure physical, psychosocial, and environmental needs and resources are integrated.
