Job summary

The Shipley Cardiothoracic Center at HealthPark Medical Center in Fort Myers, FL is seeking an experienced RN to join its triage team. Three years of RN experience and cardiology exposure are preferred. This full-time on-site role requires a Florida RN license and BLS certification, with ambulatory care certification encouraged.

You will handle referrals, triage patient inquiries, and perform post-discharge follow-ups while coordinating with the cardiothoracic surgery team to ensure timely care.

Qualifications

• Bachelor’s degree in Nursing (preferred).
• Minimum 3 years of nursing experience in relevant acute care settings.
• Florida RN license required.
• BLS certification required.
• Ambulatory Care Nurse Certification preferred.

Responsibilities

• Coordinate and manage patient referrals for the Cardiothoracic Surgery Center.
• Triage and respond to all patient caregiver calls, including preoperative inquiries.
• Conduct post-discharge follow-up calls to assess status and identify needed interventions.
• Evaluate and prioritize patient symptoms and coordinate care with providers.
• Provide patient education and support throughout the surgical continuum.
• Complete detailed clinical assessments via telephone to determine urgency and disposition.
• Utilize nursing processes and evidence-based tools to support complex decisions.
• Facilitate additional care delivery and self-care guidance as needed.

Minimum to Midpoint Pay Rate: $30.27 - $40.86 per hour

Responsibilities

• Coordinate and manage patient referrals for the Cardiothoracic Surgery Center.
• Triage and respond to all patient carerelated calls, including preoperative cardiac surgery inquiries.
• Conduct post-discharge follow-up calls to assess patient status, identify symptoms or concerns, and facilitate appropriate clinical intervention.
• Evaluate and prioritize patient symptoms, ensuring timely communication with providers and coordination of care.
• Provide patient education and support throughout the preoperative and postoperative cardiac surgery continuum.
• Complete a detailed clinical assessment via telephone to identify the urgency and nature of the patient's condition and coordinate the appropriate disposition to receive healthcare.
• Utilize nursing process and evidence‑based decision support tools; make complex decisions based on assessment, effective listening, critical thinking, advanced assessment and communication skills.
• Facilitate other care delivery and self‑care advice as the situation warrants.

Requirements

Educational Requirements

Bachelor’s degree in Nursing (preferred).

Experience Requirements

Minimum 3 years of nursing experience in emergency medicine, medical‑surgical, case management, ICU, urgent care, outpatient clinics, ambulatory care sites, or physician office. Formal or informal telephone nursing is preferred.

State of Florida Licensure Requirements

Registered Nurse License (required).

Certifications and Registration Requirements

BLS (American Heart Association / Basic Life Support) (required).

Ambulatory Care Nurse Certification (preferred).
