Intermountain Healthcare Inc. at Good Samaritan Hospital in Lafayette, Colorado seeks a Clinical Program Coordinator to oversee treatment processes and coordinate care across providers during hospitalization.
You will guide patients and families, participate in rounds, and help develop evidence-based policies. Requires CO RN licensure, BSN, 3 years RN experience, and BLS-HCP. On-site role with 40 hours/week, Monday–Friday, with potential on-call duties.
The Clinical Program Coordinator assesses, plans, implements and coordinates treatment processes from diagnosis through follow up, specifically guides patients through the collaborative process with all medical care providers and ancillary support programs and provides educational support along the continuum.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$41.20 - $62.17
