Intermountain Health is seeking a Clinical Program Coordinator (RN) at Good Samaritan Hospital in Lafayette, CO. The role involves coordinating the program, guiding patients through care with physicians and nurses, and educating patients and families during the hospital stay.
Requirements include a BSN, active RN license, BLS-HCP certification, and 3 years of RN experience. Preference for a Master's in Nursing and specialty certification.
The Clinical Program Coordinator assesses, plans, implements and coordinates treatment processes from diagnosis through follow up, specifically guides patients through the collaborative process with all medical care providers and ancillary support programs and provides educational support along the continuum.
