Job summary

Moments Hospice in Des Plaines, IL, seeks an RN Clinical Manager/Team Director to lead an interdisciplinary care team across road and clinical settings. You’ll coordinate nurses, aides, social workers, chaplains, bereavement coordinators, and volunteers to ensure holistic, regulatory-compliant hospice care.

Qualifications

• RN license required with at least 3 years in healthcare, hospice preferred.
• Bachelor's degree strongly preferred.
• Leadership or management experience is a major plus.

Responsibilities

• Lead and oversee an interdisciplinary team of nurses, aides, social workers, chaplains, and more.
• Ensure adherence to hospice regulations and standards.
• Facilitate communication between medical staff and the care team.
• Lead interdisciplinary meetings, documenting patient needs and care plans.
• Develop and update patient care policies and emergency procedures.
• Manage staff scheduling, performance reviews, and foster a positive work environment.
• Act as the central hub for communication with families, physicians, and other providers.
• Analyze quality metrics and manage a caseload of patients as needed.
• Participate in after-hours and on-call duties to ensure seamless care.
• Maximize resource utilization and promote a collaborative work culture.
