Nazareth Hospital, part of the Trinity Health family, is seeking a Registered Nurse for the Transitional Care Unit in Philadelphia. The role is per diem with a minimum of four shifts in a six-week schedule, requiring flexible coverage and strong teamwork.

The RN will assess patients, develop and adjust care plans, and work within an interdisciplinary team to deliver evidence-based, patient-centered care to diverse age groups.

Qualifications

• Graduate from an accredited nursing program; BSN preferred.
• Current RN license by PA State Board of Nursing or Compact State.
• Prior TCU, SNF, and/or long-term care experience.
• BLS certification required.

Responsabilités

• Assess patients and develop, evaluate, and modify the care plan.
• Collaborate with Care Team to achieve patient outcomes in the plan of care.
• Provide nursing care using nursing process and established standards.
• Care for patients across all ages from infant to older adulthood.

Position Purpose

The Registered Nurse – Transitional Care Unit works in a collaborative environment at Nazareth Hospital to deliver excellent patient care as part of an interdisciplinary team providing evidence-based medicine and individualized patient care. A Registered Nurse is expected to facilitate all aspects of the patient visit experience. What you will do:

• Employment Type/Shift: Per Diem Mixed/Rotation - Minimum 4 shifts in a 6 week schedule

• Under limited supervision, the Registered Nurse is responsible for assessing patients, including developing, evaluating, and modifying the care plan. The RN interprets and performs complex patient-care procedures specific to their expertise.

• RNs are members of the Care Team and Support Team and have accountability to function as a team to accomplish patient outcomes identified in the pathway/plan of care.

• The Registered Nurse is a professional caregiver who assumes responsibility and accountability for the care given to a group of patients for a designated time frame.

• The RN provides care to those patients via therapeutic use of self, the nursing process, the environment/technology, and other healthcare team members, according to established standards.

• Patients served include all age groups (Infant through Older Adults as identified in Performance Standards).

Minimum Qualifications:

• Graduate from an accredited nursing program approved; BSN preferred

• Current Registered Nurse license by the Pennsylvania State Board of Nursing or Compact State required.

• Prior TCU, SNF, and/or long-term care experience required.

• BLS required.
