The MetroHealth System (Cleveland, OH) seeks a dedicated Registered Nurse for Spry Strongsville to provide high-touch primary care and care coordination. You will join a multidisciplinary team, build patient relationships, and develop care plans while supporting daily huddles and clinical staff meetings.

Ideal candidates will have Ohio RN licensure, BLS, 2–5 years of relevant experience, and strong communication and EMR skills. This on-site role emphasizes patient experience and outcomes.

Formación

• Graduate of an accredited nursing program.
• Current RN licensure in Ohio.
• Current BLS for healthcare provider or obtained within 90 days of hire.
• 2–5 years healthcare RN experience, preferably in primary/outpatient care or care management.
• Strong interpersonal, verbal/written communication and delegation skills.
• Proficiency with data management and computer systems; leadership and organizational skills.

Responsabilidades

• Provide concierge-style nursing care to help patients navigate Spry Senior and the healthcare system.
• Build relationships with patients and manage care plans; perform outreach as needed.
• Participate in morning huddles and clinical staff meetings; coordinate clinical and auxiliary services to deliver quality care.
• Support the Spry Senior care team in decision making and uphold MetroHealth System standards.

Summary

Provides a high level of personal attention and concierge-style care to ensure patients can successfully navigate Spry Senior and the healthcare system to fulfill their healthcare needs through strong communication and service. Delivers high-quality, high-touch primary care and care coordination in conformance with approved and recognized nursing standards of care, recognized nursing techniques, procedures, and administrative policy. Builds relationships with patients, manages care plans, and performs outreach. Supports the Spry Senior care team decision making process, participates in morning huddles and clinical staff meetings, and coordinates clinical and auxiliary services to deliver a quality patient experience. Upholds the mission, vision, values, and customer service standards of The MetroHealth System and Spry Senior.

Qualifications

Required: Graduate of an accredited school of professional nursing (or a program seeking accreditation). Current Registered Nurse licensure in the State of Ohio. Current AHA Basic Life Support for the healthcare provider certification or obtains within 90 days of hire. 2-5 years of healthcare experience as an RN, preferably in primary care, outpatient care, or care management. EMR knowledge and experience. Ability to work independently and as a member of a multidisciplinary team. Demonstrates excellent interpersonal, verbal/written communication and delegation and problem-solving skills. Demonstrates strong analytical, data management and computer skills. Demonstrates strong leadership, organizational and time management skills. Demonstrates flexibility, energy, attention to detail, and problem-solving skills. Demonstrates excellence in customer experience in healthcare or hospitality industry. Desire to provide care to the people and the communities we serve. Ability to interact effectively with a wide range of cultural, ethnic, racial, and socioeconomic backgrounds. Preferred: BSN. Certification in a relevant nursing field. Knowledge of Epic EMR.

Physical Demands

Physical Demands: May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Ability to lift 250-300 pounds with assistance. Ability to push/pull up to 500 pounds with assistance. Requires some exposure to communicable diseases or body fluids. Requires some exposure to chemicals and cleaning products Ability to use the computer.
