Job summary

Signature HealthCARE of Clarksville is seeking an experienced RN to provide direct nursing care and supervise nursing staff in a 120-bed facility. You will administer medications, assist with ADLs, and develop care plans with the interdisciplinary team.

This position requires an active RN license in the state of practice, and ideally 1 year of long-term care experience. EHR familiarity and strong documentation skills are essential. Competitive benefits are offered with growth opportunities.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What You Need To Make a Difference
