Job summary

Interim HealthCare of The Upstate is seeking a dedicated Home Health Registered Nurse (RN) to provide skilled, compassionate care in patients' homes across Greenville, Piedmont, Pelzer, Williamson and surrounding areas in Anderson County.

You will perform comprehensive nursing assessments, develop and adjust plans of care, document OASIS, and collaborate with physicians and the interdisciplinary team to support safe recovery and long-term health, while maintaining patient-centered outcomes.

Qualifications

• Graduate of an accredited RN program.
• Active South Carolina RN license or Compact license.
• Minimum one year of nursing experience.
• Experience in Home Health, Medical-Surgical, Critical Care, or Case Management preferred.
• Familiarity with OASIS documentation preferred.

Responsibilities

• Provide skilled, patient-centered care in the home setting.
• Perform comprehensive patient assessments and accurate OASIS documentation.
• Develop and adjust individualized plans of care.
• Monitor patient condition and communicate changes to physicians.
• Educate patients and caregivers on medications, treatments, and home safety.
• Coordinate care with therapists, aides, and other providers.
• Prepare patients and families for discharge and document care in the EMR.

• Provide skilled, patient centered care in the home while partnering with physicians and the interdisciplinary team to support safe recovery and long-term health
• Perform comprehensive patient assessments, including accurate OASIS documentation
• Develop, implement, and adjust individualized plans of care
• Monitor patient condition and communicate changes promptly to physicians to prevent complications
• Educate patients and caregivers on medications, treatments, disease management, and home safety
• Coordinate care with therapists, aides, and other providers to ensure seamless, compliant care
• Prepare patients and families for safe, successful discharge and document care accurately in the EMR

WhatYou’llNeed

• Graduate of an accredited Registered Nurse (RN) program
• Active South Carolina RN license or Compact License
• Minimum one year of nursing experience
• Experience in Home Health, Medical Surgical, Critical Care, or Case Management preferred
