Job summary

Lakewood Rehabilitation and Healthcare is seeking a Registered Nurse Supervisor/Unit Manager to lead nursing care in a supportive, patient‑centered environment with new incentives. The RN will apply the nursing process, coordinate with physicians and teammates, and oversee unit operations on a day-to-day basis.

Qualifications include PA RN licensure, nursing degree, and current BCLS; prior skilled nursing facility experience preferred.

Qualifications

• PA RN license and eligibility to practice in Pennsylvania.
• Graduate of an accredited nursing school with an RN credential.
• High school diploma or equivalent required.
• Current AHA health care provider training (BCLS).
• Experience in a skilled nursing facility with verifiable documentation.
• Valid PA RN License held at time of application.

Responsibilities

• Providing nursing care to patients as assigned
• Administers, and records medications and treatments as assigned
• Prepares for and assists with diagnostic procedures and treatments
• Reinforces support and provides health care instructions to patients and their families
• Participates with other team members in planning, implementing, and evaluating nursing care and other patient activities
• Assists in the admission, transfer, and discharge of patients
• Assists with unit management and/or assumes charge of unit as assigned
• Utilize nursing knowledge in safe preventive, therapeutic and rehabilitative care
• Performs independent nursing functions using established policies
• Admits and assess residents including initial care plan
• Administers assessments, treatments, medications, and diets
• Completes all required resident charting every shift
• Makes rounds on unit every shift; assess orderliness and infection control
• Observes and documents changes in residents’ condition and reports to physician
• Participates in discharge planning and coordinates communication
• Directs, supervises CNAs and other nursing staff
• Works with health care team to coordinate resident care
• Performs other duties as assigned

Position Summary:

JOB DESCRIPTION:

• Utilize nursing knowledge and skills in the safe implementation of preventative therapeutic and rehabilitative nursing care of assigned resident as evidenced by documentation and observation of positive resident care outcomes.

• Performs independent nursing functions using established procedures, policies, guidelines and standards.

• Admits and assess residents including initial care plan.

• Administers assessments, treatments, medications, and diets accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes.

• Completes all required resident charting every shift.

• Makes rounds on unit every shift; assess orderliness, infection control and resident care

• Observes and documents changes in residents’ condition and report to the attending physician and other departments.

• Participates in discharge planning with physician, and other departments according to residents’ needs.

• Directs, supervises, and assists CNAs and other nursing staff in providing bedside care.

• Works collaboratively with other members of the health care team in coordination of the resident’s care as evidenced by other department feedback and coordinates communication among team.

• Performs other duties as assigned.
