Job summary

Inova Fairfax Medical Campus is seeking a Registered Nurse (RN) for the Medical/Surgical and Transitional Care Units. The role is full-time, day shift, in Falls Church, VA, with sign-on and relocation options. The hospital is part of Inova Health System, a top employer focused on safety and patient experience.

The RN will provide holistic, patient-family centered care, coordinate with multidisciplinary teams, and uphold high standards of clinical practice across the continuum.

Qualifications

• Certification: Basic Life Support (BLS) required.
• Licensure: RN licensed in Virginia or eligible for licensure.
• Experience: 1 year RN experience; critical care experience preferred.
• Education: BSN or ADN; BSN must be obtained within 5 years if ADN.

Responsibilities

• Performs comprehensive patient health assessments and synthesizes data.
• Develops individualized plans of care based on assessment findings.
• Delivers patient- and family-centered care across the continuum of care.
• Collaborates with interdisciplinary team to coordinate care and transitions.
• Documents all patient care activities and outcomes in the electronic health record.
• Supports patient education and measures learning outcomes.
• Provides clinical guidance and mentorship to staff as needed.

The Registered Nurse provides knowledgeable and caring clinical practice and care coordination through an understanding of patients, families, nurses and healthcare delivery teams. Consistently provides safe and therapeutic care in a holistic and systematic way while incorporating differences into the provision of care and conducting all patient care in a patient/family centered manner. Integrates knowledge, skills and experiences to meet the needs of patients and families throughout the continuum, to include patient and family education. Communicates effectively and works cooperatively with others while having respect for and an understanding of other clinical disciplines. Determines processes for delivery of care from direct patient/family communication, technology and other healthcare team member collaboration.

Job Responsibilities

• Performs comprehensive patient health assessments by interpreting multiple data sets and synthesizing their interrelationships
• Formulates individualized plans of care based on assessment findings and interpreting trends in patient populations to achieve best practice
• Conducts all patient care in a patient and family centered manner, integrating knowledge, skills and experiences to meet the needs of patients and families throughout the continuum of care
• Advocates for patients' rights from the patient and family perspective and provides counsel while acting as a resource to resolve issues
• Accesses resources from all disciplines to achieve desired patient outcomes and coordinates healthcare and alternate services for patient/family transition planning
• Documents all patient care activities, as per documentation standards, and assists healthcare team members with documentation
• Evaluates the process to achieve outcomes, anticipates patient variances and makes revisions to plans of care
• Makes clinical decisions based on experience, patient population data and intuition while promoting the development of clinical decision making of all team members
• Communicates effectively, works cooperatively with others, and has respect for and an understanding of other clinical disciplines
• Assists staff to assess patients' learning needs and outcomes measurement of patient education
• Provides direction and guidance to others regarding practice while serving as a clinical resource, preceptor, and mentor and delegates patient care activities and coordinates unit activities as needed
• Utilizes standards, guidelines and pathways for care delivery and ensures that the process for delivery of care is synthesized from direct patient/family communication, technology and other healthcare team member collaboration
• Demonstrates evidence of self-development by obtaining a minimum of six formal contact hours and maintains own Team Member Continuing Education Record
• Evaluates clinical and quality outcomes and contributes to performance improvement initiatives
• May perform additional duties as assigned

Requirements

• Certification: Basic Life Support (American Heart Association)
• Licensure: Licensed or eligible for licensure in the Commonwealth of Virginia as a Registered Nurse or an active multi-state Registered Nurse license
• Experience: 1 year of experience as a Registered Nurse, for Critical Care units, 1 year of experience as a Registered Nurse in a critical care setting or completion of a critical care fellowship
• Education: Bachelor's Degree Nursing or Associate's Degree. If RN has an Associate's Degree, must complete BSN within 5 years of start date.
• Preferred qualifications: 1-2 years of experience as Medical/Surgical RN
