Prisma Health in Greenville Memorial Hospital seeks a Registered Nurse for telephone triage to provide patient care guidance and education by phone. You will assess symptoms, determine appropriate care level, and guide patients to the right care pathway, while supporting care coordination and appointment scheduling.

In this role you will document assessments, collaborate with the multi-disciplinary team, and follow evidence-based guidelines to ensure timely, safe care for patients of all ages.

Qualifikationen

• Associate degree in Nursing required; Bachelor’s degree in Nursing preferred.
• Two years in Acute Care Nursing.
• RN license recognized by the state.

• Provides education, home-care instructions, or follow-up recommendations per departmental processes and guidelines.
• Schedules appointments for patients
• Maintains thorough, accurate documentation of patient assessment and recommendations.
• Collaborates with clinicians or other members of the multi-disciplinary care team
• Performs other duties as assigned.

Supervisory/Management Responsibilities

• This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

• Education - Associate degree in Nursing. Bachelor’s degree in Nursing preferred
• Experience - Two (2) years in Acute Care Nursing. Experience with Schmitt-Thompson guidelines or Telephone Triage experience preferred

• Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.
• Certified in nursing specialty - Preferred
