Job summary

Signature HealthCARE is seeking a skilled registered nurse for our Monroe County Rehab & Wellness Center. The role emphasizes direct patient care, wound care, and medication administration within a supportive team environment.

You will supervise CNAs and aides, monitor residents' conditions, and collaborate with an interdisciplinary team to update care plans. Strong documentation and regulatory knowledge are essential for success.

Qualifications

• Current, active RN license in the state of practice.
• Experience in a supervisory or charge nurse role is preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living while ensuring dignity and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' condition and communicate changes to physicians and families.
• Develop and review resident care plans with the interdisciplinary team.
• Maintain accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in QA activities, staff education, and surveys as needed.
• Perform other duties assigned by the Director of Nursing.
