Job summary

Northwell Health’s Danbury Hospital Rehab Unit is seeking a part-time Registered Nurse to provide professional care and coordinate a plan of care for a patient group. You will work with a multidisciplinary team to deliver high-quality nursing care, performing assessments, planning, and evaluating patient outcomes.

The role requires CT RN license, BLS, and at least one year of acute care experience. This position is in Danbury, CT and supports a standard 24-hour weekly schedule as listed.

Qualifications

• State of CT RN license required.
• BLS certification required.
• One year of RN Acute Care experience required.

Responsibilities

• Performs assessments in an ongoing and systematic manner.
• Formulates a goal directed plan of care based on nursing diagnosis and patient outcomes.
• Implements care in a knowledgeable, skillful, continuous manner.
• Establishes priorities of patient care based on essential needs and available resources.
• Evaluates effectiveness of care and patient progression toward outcomes.
• Identifies patient learning needs and implements appropriate measures.

Shift: 7:00am-3:30pm; including every other weekend Saturday & Sunday

Summary

Provides professional care and assumes responsibility and accountability for a group of patients for a designated time frame. Provides care to these patients utilizing the nursing process in collaboration with other health care team members.

• Identifies patient's/significant other's learning needs and implements appropriate measures to meet them.

• Performs other duties as assigned.
