Job summary

Avamere in Lake Oswego, OR is seeking a Registered Nurse (RN) for PRN shifts in a Skilled Nursing setting. This role covers Day and Night 12-hour shifts, with supervision of LPNs/CNAs and coordination of comprehensive resident care at The Pearl at Kruse Way.

Ideal candidates hold an Oregon RN license, a nursing degree, and CPR/BLS certification, with preferred BSN and long-term care experience. Avamere offers a strong benefits package and opportunities for professional growth.

Qualifications

  • Must possess a nursing degree from an accredited college or university.
  • Must possess a current, active RN license in Oregon (BSN preferred).
  • CPR/BLS current during employment.
  • Experience in long term care, geriatric rehab, sub-acute or critical care settings preferred.
  • Experience with electronic medical records documentation.

Responsibilities

  • Assess residents, develop and update individualized care plans with interdisciplinary team.
  • Administer medications and monitor residents’ responses for safety and accuracy.
  • Supervise and train LPNs and CNAs, provide orientation to new staff.
  • Document all care accurately and communicate with residents, families and other health professionals.
  • Participate in quality improvement initiatives and stay current with best practice guidelines.

Setting: Skilled Nursing

Statue: PRN

Shift: Days (6:00 AM to 6:00 PM) and NOCs (6:00 PM to 6:00 AM) 12-hour shifts

PRN Available ALL Shifts

Location: The Pearl at Kruse Way, 4550 Carman Drive, Lake Oswego, Oregon 97035

The primary responsibility of the Registered Nurse (RN) is to provide comprehensive patient care, including assessing residents, administering medications, monitoring health, and coordinating care with other healthcare professionals, while also supervising nursing staff.

Essential Duties and Job Responsibilities

  • Patient Assessment and Care Planning:
  • Conduct thorough assessments of residents' physical, cognitive, and emotional needs.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team, including physicians, therapists, and other healthcare professionals.
  • Regularly evaluate and update care plans based on residents' changing needs and responses to interventions.
  • Medication Administration and Monitoring:
  • Administer medications and treatments as prescribed by physicians, ensuring accuracy and safety.
  • Monitor residents' responses to medications and treatments, documenting any adverse effects or changes in condition.
  • Vital Signs and Health Monitoring:
  • Monitor and document vital signs, such as temperature, pulse, respiration, and blood pressure.
  • Observe residents for any signs of illness or changes in condition and promptly report any concerns to the physician.
  • Wound Care and Other Specialized Care:
  • Collaborate with wound care specialists or other healthcare professionals to ensure optimal wound healing.
  • Supervision and Training:
  • Supervise and provide guidance to licensed practical nurses (LPNs) and certified nursing assistants (CNAs).
  • Provide orientation and training for new staff members.
  • Communication and Documentation:
  • Maintain accurate and complete documentation of residents' care, including assessments, interventions, and evaluations.
  • Communicate effectively with residents, families, and other healthcare professionals.
  • Participate in interdisciplinary team meetings to coordinate care and address resident needs.
  • Quality Improvement:
  • Participate in quality improvement initiatives to ensure the delivery of high-quality care.
  • Stay current with best practices and evidence-based guidelines for nursing care in skilled nursing facilities.
  • Must possess a nursing degree from an accredited college or university.
  • Must possess a current, unencumbered, active license to practice as an RN in this state. BSN preferred.
  • Must maintain and have an active CPR/BLS during employment.
  • Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting.
  • Experience in electronic medical records documentation.
