BAYADA Home Health Care, Inc. is seeking an experienced RN Preceptor to train newly hired RNs and LPNs while maintaining a caseload of patients. The role emphasizes hands-on nursing, mentorship, and ongoing education within a Bayhealth setting.

Qualifications

• Active RN license required.
• Prior Medicare-Certified Home Health experience required.
• Strong clinical skills including wound care, IVs, ostomies, feeding tubes.
• Experience in teaching, coaching, and mentoring preferred.

Responsibilities

• Precept newly hired RNs and LPNs, providing one-on-one clinical training during home visits and mentoring through the first year.
• Monitor and manage orientation progression, identify areas for growth.
• Provide 1:1 mentoring and feedback to new field clinicians, in person or via Zoom.
• Lead Quality Assurance Performance Improvement (QAPI) initiatives.
• Collaborate with RN Clinical Manager and Educator to ensure staff competencies.
• Onboard new clinicians to help them build and manage their own caseloads.
• Case manage and admit new clients while precepting clinicians and performing RN duties.
• Document observations, interventions, and evaluations in tablet-based system.

BAYADA at Bayhealth is looking for a Full-Time RN Preceptor to lead the training and development of newly hired RNs and LPNs — while continuing to make a hands-on difference with a caseload of your own patients.

Prior Medicare-Certified Home Health experience is required, with HCHB experience preferred.

What You'll Do:

Precept & Mentor

• Precept newly hired RNs and LPNs, providing one-on-one clinical training during home visits and serve as their mentor through their first year.
• Monitor and manage progression of orientation, identify and address areas of opportunity.
• Provide 1:1 mentoring, support, and meaningful feedback — in person or via ZOOM — to new field clinicians
• Quality Assurance Performance Improvement (QAPI) Lead

Educate & Onboard

• Collaborate with the RN Clinical Manager and Clinical Educator to ensure ongoing staff competencies
• Onboard new clinicians to help them build and manage their own caseloads

Deliver Direct Care

• Case manage and admit new clients while precepting and mentoring clinicians
• Perform the duties of an RN Case Manager, including medication administration, wound care, treatments, and procedures
• Complete client assessments as needed
• Monitor client conditions and report changes to the Clinical or Client Services Manager
• Follow up on, execute, and accurately document physicians' orders
• Document all observations, interventions, and evaluations using a tablet-based point-of-care system

Support the Bigger Picture

• Work closely with the office team, Clinical Education team, and fellow preceptors to keep training consistent and cohesive
• Partner with the office and Clinical Education team to help monitor and improve clinician retention
• Leadership Track: Seeking a motivated candidate with a strong desire for professional growth and an interest in advancing into a future leadership role

What You Bring:

• Active RN license
• Prior Medicare-Certified Home Health experience — required
• Strong clinical skills i.e. wound vacs, IV’s, ostomies, feeding tubes, complex clinical care
• A genuine passion for clinical education, coaching, and mentorship, with a strong desire for continued professional growth and career advancement
• Strong clinical judgment and the confidence to lead by example
• Excellent communication, organization, and relationship-building skills

Compensation:

Base Pay, depending on qualifications, and subject to negotiation:

$85,000 - $90,000 annual earning potential – paid weekly
