Job summary

Commonheart in Fond du Lac, WI is seeking a compassionate RN or LPN to join our hospice team and deliver exceptional end-of-life care.

You will work with an interdisciplinary team to develop individualized care plans, educate families, and advocate for patients while documenting care and monitoring changes in condition. This full-time role includes on-call shifts and a one-hour travel radius with a company car for 0.6 FTE or more.

Qualifications

• Current RN or LPN license.
• Excellent communication and interpersonal skills.
• Hospice, home health, or long-term care experience preferred but not required.
• Valid driver's license and reliable transportation.

About The Role

What You'll Do

Schedule

• Monday - Friday: 8:0 AM to 4:30 PM
• 0.8 FTE (32 hours a week), or 1.0 FTE (40 hours a week)

On-Call Requirement

• This role includes on call coverage.
• Up to 1 shift per week, Monday - Thursday 4 PM to 8 AM
• Up to every 4th weekend, Friday 4 PM to Monday 8 AM

Location

• Hit the road without the extra costs—this role includes travel within a one-hour radius, and a company car is provided for employees working 0.6 FTE or more.

Compensation

• LPN: $27.80–$31.20 per hour, based on experience.
• RN: $38.50–$44.80 per hour, based on experience.

(Candidates with exceptional qualifications and relevant experience may be considered for a higher rate)
