Job summary

Hartford HealthCare's St. Vincent's Medical Center in Bridgeport, CT, is seeking an experienced RN for a 36-hour evening position with weekend and holiday call requirements. The role involves coordinating multi-disciplinary care and implementing individualized patient care plans in a high-volume hospital setting.

Current CT RN license is required; BSN preferred. BLS/ACLS/PALS certifications and PACU/ICU/ED experience are highly valued as we deliver compassionate, high-quality care to diverse

Qualifications

• Licensed as RN in CT.
• BSN preferred from an accredited program.
• Current BLS, ACLS, and PALS certifications.
• PACU and/or ICU/ED experience preferred.

Responsibilities

• Implements and monitors patient care plans.
• Serves as primary coordinator of all disciplines for well-coordinated patient care.
• Notes and carries out physician and nursing orders.
• Assesses discharge planning needs with the healthcare team.
• Demonstrates ability to assess diverse patient needs and provide individualized care.
• Delegates care tasks to nursing staff to meet patient needs.
• Participates in daily interdisciplinary rounds and team input for outcomes.
• Adheres to ANA standards of practice and ethics.
• Develops and revises individualized care plans.
• Pursues professional growth activities beyond direct patient care.

Responsibilities

• Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate.
• Serves as a primary coordinator of all disciplines for well-coordinated patient care.
• Notes and carries out physician and nursing orders.
• Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
• Demonstrates the knowledge and skills necessary to assess patients and to identify physical, psychosocial, cultural, spiritual and age-related needs in order to provide individualized care to the patient.
• Delegates aspects of care to other nursing personnel consistent with their level of education and expertise in order to meet patients' needs and to maximize staff performance.
• Participates in daily interdisciplinary patient rounds. Demonstrates knowledge of patients' condition and is able to provide appropriate
• input to meet desired outcomes for the appropriate patient population.
• Reflects ANA Scope and Standards of Nursing Practice (2015), ANA Code of Ethics (2015), and ANA Social Policy Statement (2015).
• Develops, implements and revises individualized plans of care.
• Participates in activities for professional growth and development outside of patient care.

Qualifications

• Current CT RN license
• Graduate of accredited nursing school with BSN preferred
• Current BLS, ACLS, PALS
• PACU and or ICU/ ED experience preferred
