Community Health Network is seeking a Primary Care Navigator in Indianapolis, Indiana, to join a multi-disciplinary team. The role involves timely identification of patients’ needs, educating on disease processes, and coordinating services to support transitions across levels of care.
Qualified candidates require at least two years of oncology nursing experience and a valid Registered Nurse license in Indiana. Additional preferences include OCN certification and experience with electronic medical records (EMR).
The Primary Care Navigator will work as part of a multi‑disciplinary team led by the PCP to ensure timely identification of patients’ needs and coordination of patient services. They will educate on disease processes, set goals, and monitor patients identified for wellness, transition, chronic, and high‑risk care management programs. The Navigator will help facilitate members’ access to services throughout the healthcare continuum and support transitions between levels of care.
