Job summary

Community Health Network is seeking a Primary Care Navigator in Indianapolis, Indiana, to join a multi-disciplinary team. The role involves timely identification of patients’ needs, educating on disease processes, and coordinating services to support transitions across levels of care.

Qualified candidates require at least two years of oncology nursing experience and a valid Registered Nurse license in Indiana. Additional preferences include OCN certification and experience with electronic medical records (EMR).

Job description

The Primary Care Navigator will work as part of a multi‑disciplinary team led by the PCP to ensure timely identification of patients’ needs and coordination of patient services. They will educate on disease processes, set goals, and monitor patients identified for wellness, transition, chronic, and high‑risk care management programs. The Navigator will help facilitate members’ access to services throughout the healthcare continuum and support transitions between levels of care.

Responsibilities

• Work as part of a multi‑disciplinary team led by the PCP for timely identification of patients’ needs and coordination of patient services.
• Educate members on disease processes, set goals, and monitor patients identified for wellness, transition, chronic, and high‑risk care management programs.
• Facilitate member access to services throughout the healthcare continuum and support seamless transitions across levels of care.

Qualifications

• Two years or more of oncology nursing experience required.
• OCN certification preferred, but not required upon hiring. OCN or ONN‑CG certification required within a timeframe established upon hire.
• Three years of nursing experience; five years preferred.
• Preferred experience in case/care management role, or in home health or other independent nursing roles requiring assessment, critical thinking, and application.
• Experience with electronic medical records (EMR) is a serious plus.
• Clinical background and genuine interest in or connection to the healthcare industry is a serious plus.
• Certified Case Manager (CCM) preferred.
• Graduate of an accredited nursing school (NLN, CCNE, ACEN, NLN CNEA) or have three years of related professional nursing experience.
• Licensed as a Registered Nurse (RN) with a valid Indiana license under the Nurse Licensure Compact (NLC).
• BSN preferred.
