Nazareth Hospital, part of Trinity Health Mid-Atlantic, seeks a Registered Nurse to deliver high-quality patient care on a 12 Hour Night Shift. The RN will assess, plan, implement, and evaluate care, lead a care team, and ensure safety and comfort for patients and families.

The role requires strong clinical judgment, excellent communication, and commitment to holistic nursing. Eligible candidates must hold an active PA RN license, BSN preferred, and participate in ongoing education.

Formación

• BSN or BS/BA in Nursing is required.
• Current RN Pennsylvania state license is required.
• BCLS certification required or obtained by orientation.
• ACLS certification required or obtained if applicable.
• Professional certifications are preferred.

Responsabilidades

• Provides total, comprehensive patient care using the nursing process.
• Communicates plan of care and changes in patient condition to the health care team.
• Documents assessments, plans, interventions and outcomes per policy.
• Documents pain, fall risk and wound risk assessments and plans.
• Performs accurate nursing physical assessments.
• Participates in discharge planning with a multidisciplinary team.

Shift: Full Time Nights

The Registered Nurse (RN) is responsible for assessing, planning, implementing and evaluating the delivery of patient care. The professional nurse assumes the responsibility and accountability for the delegation of patient care to other members of the healthcare team.

• Provides total, comprehensive care to patients by applying the nursing process in an organized and systemic manner according to hospital policy.
• Communicates proactively to the health care team, plan of care, changes in patient condition, the patient progress, the discharge plan and other relevant information.
• Documents assessment/reassessment, plan, interventions and outcomes according to policy and interprets and utilizes clinical data in the plan of care:
• Documents pain assessment and plan of care.
• Documents fall risk and plan of care
• Documents wound risk and plan of care.
• Interprets & utilizes clinical data in the plan of care.
• Assesses patient for potential risk factors.
• Performs accurate and thorough nursing physical assessment.
• Participates in discharge planning, using a multi-disciplinary approach.

Education and Training

• BSN or BS/BA degree in Nursing Certification and Licensure
• Current RN Pennsylvania state license required.
• Current BCLS required or obtained by completion of orientation.
• ACLS required or obtained by completion of orientation if Applicable to Area of Practice.
• Professional certifications preferred.

Experience

• Prior clinical experience preferred
