Job summary

Altra Homecare Group is seeking a dedicated Registered Nurse (RN) to deliver skilled home health services in Indiana. You will provide patient-centered care in patients' homes while ensuring Medicaid compliance and high-quality documentation.

Ideal candidates hold an Indiana RN license, at least 1 year of nursing experience (home health/Medicaid preferred), CPR, and a valid driver’s license. You will collaborate with care teams and coordinate with physicians to support health and independence.

Qualifications

• Current and unrestricted RN license in Indiana.
• At least 1 year of nursing experience; home health and Medicaid experience preferred.
• Strong understanding of Indiana Medicaid policies, documentation standards, and prior authorization processes.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Sharp clinical judgment along with strong communication and organizational skills.
• Familiarity with electronic documentation systems.

Responsibilities

• Provide skilled nursing services, including wound care, medication administration, chronic disease management, and patient education.
• Ensure all services are medically necessary and properly documented to meet Medicaid reimbursement standards.
• Coordinate care with physicians, case managers, and other members of the interdisciplinary team.
• Empower patients and caregivers with knowledge on disease processes, medication regimens, and self-care strategies.
• Maintain accurate, timely, and compliant documentation within the electronic health record (EHR).
• Take part in Medicaid audits, quality assurance initiatives, and case conferences.
• Champion patient needs and help connect them with community resources and support services.
