Position Summary

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and standards of professional nursing practice. The nurse will operate within the framework for professional nursing practice and development, and is accountable for the quality of nursing services delivered by self or others under their direction. The nurse utilizes specialized knowledge, judgment and nursing skills to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.

Core Functions
  • Assess patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs. Review patient history and physical with patient/family and ensure completion within the appropriate timeframe. Review information from other health care team members. Review diagnostics and laboratory data and report abnormal results to the physician and other caregivers. Complete assessments and reassessments as needed and per policy.
  • Formulate a plan of care, including discharge planning, using assessment data and input from the patient, family and health team. Initiate a care plan based on patient-specific needs and the medical plan of care. Establish realistic, measurable goals in collaboration with the patient/family. Plan care in collaboration with multidisciplinary team and revise as the patient’s status changes.
  • Implement the plan of care through direct patient care, coordination, delegation and supervision of the health care team. Follow physician orders and nursing plan of care in accordance with policies, standards of care and regulatory requirements. Delegate appropriately and supervise care provided by other licensed and support staff. Communicate patient status accurately to other caregivers to promote continuity of care.
  • Evaluate the patient’s and family’s response to care and teaching; revise the plan of care as needed. Assess progress toward goals and outcomes in collaboration with the health care team. Assess the effectiveness of patient teaching.
  • Document assessment, planning, implementation and evaluation in the patient record. Ensure documentation is legible, timely and in accordance with policy. Document objective/subjective data, nursing interventions and patient response. Note physician orders accurately and promptly.
  • Provide care based on best available evidence and participate in research activities within clinical practice. Participate in unit or facility shared leadership. Contribute to education, role development and orientation of facility personnel, patients, families and visitors. Support growth of others through precepting and mentoring when appropriate.
  • Contribute to outcomes through safe, efficient, equitable patient care. Participate in quality improvement activities and professional organizations as appropriate.
  • In some roles, supervise staff and workflow within the department.
  • Interact with all levels of staff, physicians, patients and external contacts to coordinate and report on patient health status. Work with clergy to provide spiritual support. Synthesize and prioritize data from multiple sources to support patient and family responses to changes in health status.
Minimum Qualifications
  • Current, valid RN license in the state of practice, temporary/interim RN license, or compact RN licensure for the current state of practice. BLS certification required. Additional certification or continuing education may be required based on area of practice.
  • Experience in the clinical area for which applying is desired. Must maintain clinical performance competencies with annual validations. Banner Registry and Travel acute care positions require a minimum of one year of acute care experience, including the past 12 months in the specialty area. Banner Registry and Travel physician practice positions require one year in a physician practice or acute care setting.
  • In Home Health settings, the ability to operate a personal vehicle and maintain a valid driver’s license; auto insurance active and maintained.
  • Some Banner roles in behavioral health settings may require specific clearance cards as per location and role requirements.
Preferred Qualifications
  • Bachelor’s degree preferred. Professional certification preferred.

Estimated Pay Range: $33.11 - $55.18 per hour. Banner Health is committed to pay equity and transparency. The posted compensation range is an estimate and may vary based on experience, education, certifications, skills and location.
