Job summary

VALLEY HOUSE INC in Baltimore is seeking a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join a patient-centered care team. You will manage individual care plans, observe and communicate effectively for patients, and ensure current nursing practices are applied with attention to detail.

The role emphasizes coordination with clinical staff, medication management, and health education for clients, including screening for communicable diseases and helping patients access primary care.

Qualifications

• Must be currently licensed by the Maryland Board of Nursing as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).

Responsibilities

• Oversee medication administration record (MAR) to ensure client medication management
• Monitor medicated assisted treatment (MAT)
• Working cohesively with clinical staff to provide optimal care for our clients
• Screening clients for communicable diseases
• Updating client health information/ Assist clients in finding primary care physician (PCP)
• Monitoring and maintaining medical supply inventory
• Assisting in the development and implementing of treatment strategies
• Provide educational group sessions on Men's Health

Job Summary

We are seeking a Registered Nurse/ License Practical Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) or License Practical Nurse (LPN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should be able to demonstrated knowledge and skill in current nursing practices as well as the ability to communicate the importance of maintaining good health care.
