Job summary

Avamere Keizer in Keizer, OR is hiring a Registered Nurse (RN) for a full-time day shift. You will assess residents, develop care plans, and collaborate with an interdisciplinary team. Responsibilities include medication administration, monitoring vital signs, wound care, staff training, documentation, and quality improvement.

Requires active RN license in Oregon and a nursing degree. BSN preferred; CPR/BLS current; experience in long-term care or sub-acute settings valued; EMR documentation

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, unencumbered, active license to practice as an RN in Oregon.
• BSN preferred.
• Must maintain and have an active CPR/BLS during employment.
• Prefer experience in long term care, geriatric rehab, sub-acute and or critical care settings.
• Experience in electronic medical records documentation.

Responsibilities

• Assess residents' physical, cognitive, and emotional needs and develop individualized care plans in collaboration with the interdisciplinary team.

• Monitor vital signs and report concerns promptly to physicians.

Registered Nurse | RN Status: Full Time Location: Avamere Keizer: 5210 River Rd North, Keizer, OR

Essential Duties and Job Responsibilities

• Patient Assessment and Care Planning: Conduct thorough assessments of residents' physical, cognitive, and emotional needs. Develop and implement individualized care plans in collaboration with the interdisciplinary team, including physicians, therapists, and other healthcare professionals. Regularly evaluate and update care plans based on residents' changing needs and responses to interventions.
• Medication Administration and Monitoring: Administer medications and treatments as prescribed by physicians, ensuring accuracy and safety. Monitor residents' responses to medications and treatments, documenting any adverse effects or changes in condition.
• Vital Signs and Health Monitoring: Monitor and document vital signs, such as temperature, pulse, respiration, and blood pressure. Observe residents for any signs of illness or changes in condition and promptly report any concerns to the physician.
• Wound Care and Other Specialized Care: Collaborate with wound care specialists or other healthcare professionals to ensure optimal wound healing.
• Supervision and Training: Supervise and provide guidance to licensed practical nurses (LPNs) and certified nursing assistants (CNAs). Provide orientation and training for new staff members.
• Communication and Documentation: Maintain accurate and complete documentation of residents' care, including assessments, interventions, and evaluations. Communicate effectively with residents, families, and other healthcare professionals. Participate in interdisciplinary team meetings to coordinate care and address resident needs.
• Quality Improvement: Participate in quality improvement initiatives to ensure the delivery of high-quality care. Stay current with best practices and evidence-based guidelines for nursing care in skilled nursing facilities.
