Job summary

Quail Run Hospital in Phoenix, AZ seeks an Intake and Assessment Nurse to evaluate medical and behavioral needs, respond to referrals, and coordinate patient assessments and admissions. The role may include Team Lead responsibilities at the Director of Admissions' discretion.

The RN will triage calls, gather essential information, perform initial assessments, and ensure timely communication with families, referral sources and unit staff. Strong crisis intervention skills are valued.

Qualifications

• Active Arizona nursing license or compact privileges.
• Prior intake/assessment experience in a psychiatric setting preferred.
• BLS for Providers; Level 1 Arizona Fingerprint Clearance required or able to obtain within 30 days of hire.

Responsibilities

• Answer calls with appropriate greeting and collect necessary clinical, demographic, and insurance information.
• Conduct thorough medical evaluation to support admission process and communicate findings to the facility.
• Perform initial clinical assessment including treatment history and diagnosis for admitted patients.
• Notify unit staff of pending admissions and provide necessary demographics.
• Schedule assessments, psychiatric evaluations and admissions within 24 hours and explain process to patients/referral sources.

Job Description

The Intake and Assessment Nurse is a registered professional nurse who is responsible for assessing the medical and behavioral needs of the patient. The Intake and Assessment Nurse assists in the process of responds to referrals, schedules patient assessments/appointments, triages patients, and maintains communication with referral sources, families, patients and unit staff. May be assigned Team Lead role at the discretion of the Director of Admissions

Job Duties/Responsibilities

• Answers telephone within 3 rings using the appropriate greeting and attempts to obtain all pertinent clinical, demographic and insurance information from the caller to determine caller’s needs. Keeps aware of calls waiting in the queue which require immediate attention.
• Completes a thorough and appropriate medical evaluation within the scope to support the admission process, ensuring all required clinical information is accurately gathered, documented, and communicated to facility in a safe and timely manner.
• Conducts an initial clinical assessment on all individuals presenting for admission, which includes treatment and medication trial history, family dynamics, etc., and establishes an initial diagnosis for patients who are admitted.
• Notifies unit staff of pending admission and provides pertinent demographic information.
• Schedules assessments, psychiatric evaluations and admissions with potential patient/referral source within 24 hours. Explains the evaluation and admission process to the potential patient/referral source and informs patient of necessary items to bring for hospital stay.

Qualifications

Job Requirements

• Current license to practice nursing in Arizona or compact privileges (out of state compact license: must apply for an AZ license within 60 days of hire).
• Prior intake, assessment, admission, and crisis intervention experience with a psychiatric mental health population, preferably in an inpatient setting strongly preferred.
• Handle With Care and Basic Life Support (BLS) for Providers required. Training provided at the facility.
• Level 1 Arizona Fingerprint Clearance card or be able to obtain within 30 days of hire.
