Job summary

AdventHealth in Orlando, FL is seeking an experienced nurse-care manager to coordinate discharge planning and ensure timely transitions for patients across inpatient and post-acute settings.

You will work with multidisciplinary teams to align clinical, social, and financial needs, engaging families and caregivers to optimize outcomes.

Qualifications

  • Nursing degree required; RN licensure
  • Associates of Nursing is required; a Bachelor's degree is preferred
  • Experience in hospital care management/utilization management is a plus
  • Strong leadership, analytical, and communication skills are essential
  • Ability to work with diverse patient populations and care teams

Responsibilities

  • Participates in multi-disciplinary rounds to review patient status, need resources, and discharge plans.
  • Educates patients and families about emotional, social, and financial aspects of illness and mobilizes community resources.
  • Assesses readmitted patients to understand reasons and plan action.
  • Organizes and facilitates patient and family care conferences with the team.
  • Documents discharge planning evaluations, progress, barriers, and required authorizations for post-acute care.
  • Assesses holistic discharge needs including supports and finances to avoid delays.
  • Reviews medical records and integrates clinical and social factors into discharge plans.
  • Develops discharge plans with contingency steps and coordinates post-acute care arrangements.
  • Leverages technology to ensure complete records and accurate discharge reconciliation.
  • Performs other duties as assigned.

Job Description

AdventHealth will bring a 60-acre health and wellness campus to Lake Nona in late 2026. This location will offer medical innovations on a campus designed around whole person health with comprehensive services, destination lifestyle programs, and much more that will shape the next generation of health care. We are proud to be a part of the growing Lake Nona community with an established comprehensive network of care. Our services include emergency services, urgent care, a wide range of medical specialists (including primary care), lab, imaging, orthopedics, sports medicine and rehabilitation. Shift: Days Schedule: Monday-Friday 40 hours 8:30AM-5PM with Weekend Rotation

  • Actively participates in multi-disciplinary rounds to review changes in patient status, progression and level of care, and discharge plans for all assigned patients to identify resources necessary at discharge and ensure a timely transition, escalating care delays to leadership as appropriate.
  • Communicates with and educates patients and families regarding emotional, social, and financial impacts of illness and mobilizes family/community resources to meet identified needs while advocating for patient and family empowerment in making health care decisions and accessing needed services.
  • Assesses readmitted patients for the patient’s and family’s perceived reasons for the readmission.
  • Organizes and facilitates patient and family care conferences with the multidisciplinary team.
  • Documents discharge planning evaluation, ongoing assessment, discharge plans, MDRs, barriers to progression of care, avoidable days, and patient and family needs according to standard work. Communicates with Payors patient’s needs for authorization for post-acute care as needed.
  • Assesses patients’ and families’ wholistically for discharge planning needs in the inpatient, observation and/or emergency departments, including prior functioning, support systems, financial, and psychosocial in a timely fashion to avoid delays in discharge planning.
  • Reviews the medical record, including medications, history and physical, labs, and progress notes and incorporates the clinical, social, and financial factors into the transition of care plan.
  • Develops discharge plans with appropriate contingency plans throughout the hospital stay to ensure timely care coordination and progression of care, making arrangements for post-acute care services and facilities as well as community care for social needs.
  • Leverages technology and follows standard work and best practices to communicate with post-acute care services and facilities to ensure patient care information is communicated for continuity of care, medical records are complete, and discharge reconciliation is accurate.
