Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to provide skilled care in patients' homes in the Ogden, Utah area. The role involves delivering care per physician orders and plan of care, and teaching patients and families to support ongoing health at home.

Ideal candidates have at least one year of licensed experience, CPR certification, and basic tech skills for mobile devices. A valid state license and reliable transportation are required; Medicare home health or hospice

Qualifications

• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must possess CPR certification for the healthcare provider.
• Must possess basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).

Responsibilities

Education And Experience, Preferred

• Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director.


• Medicare home health or hospice experience is preferred.

• Must maintain automobile liability insurance as required by law


• Must maintain dependable transportation in good working condition


• Must be able to safely drive an automobile in all types of weather conditions


• Must possess CPR certification for the healthcare provider
