Job summary

Enhabit Home Health & Hospice is seeking qualified healthcare professionals to deliver skilled home-based care in a collaborative team. You will administer services under physician orders, educate patients and families, and coordinate care within the plan of care to ensure high-quality outcomes.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.
• Experience as a licensed professional may be deferred with regional approval.
• Medicare home health or hospice experience is preferred.

Responsibilities

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).

• Must maintain automobile liability insurance as required by law


• Must maintain dependable transportation in good working condition


• Must be able to safely drive an automobile in all types of weather conditions


• Must possess CPR certification for the healthcare provider
