Job summary

Enhabit Home Health & Hospice is seeking a licensed professional to provide skilled home health care to patients in the community. You will administer nursing services, educate families, and collaborate with the care team to follow the physician's orders and the plan of care.

The role requires an active state license and at least one year of professional nursing experience, plus basic tech skills including mobile device use. Valid driver’s license and reliable transportation are expected.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where you practice.
• At least one year of experience as a licensed professional.
• Basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer skilled care to patients per physician orders and plan of care.
• Educate patient, family and health-care team members.
• Follow established plan of care and documentation requirements.

Responsibilities

Administer skilled care to patients requiring intermittent professional services. Teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC).

Qualifications

• Must be a graduate of an approved school of clinical education. Must be licensed in the state where they currently practice. Must have at least one year experience as a licensed professional.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education And Experience, Preferred

• Experience as a licensed professional may be deferred with a review and approval from regional leadership.
• Medicare home health or hospice experience is preferred.

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification
