AccentCare in Marble Falls, TX is hiring an RN Case Manager, Home Health. The role includes delivering quality patient care according to the Patient Care Plan, supervising CNAs, and coordinating care across pediatric to geriatric patients. Schedule: Monday–Friday 8am–5pm; no on-call.

Requirements: active RN license in TX, at least 1 year of nursing experience, and current CPR. This full-time position is paid on a per-point basis with compensation reflecting experience.

Qualifications

• Registered Nurse with current licensure to practice nursing in the state.
• Registered Nurse with minimum (1) year of experience.
• Possesses and maintains current CPR certification.

Responsibilities

• Assessment of the status of nursing needs of assigned patients, implements the individualized patient care plan, evaluates its effectiveness and revises as needed.
• Promotes and maintains optimal client care; diagnoses and provides treatment for health problems.
• Initiates the plan of care and necessary revisions; supervises and teaches other clinical personnel.

An RN Field Nurse is responsible for maintaining the delivery of quality patient care by assuring that the Patient Care Plan is followed. The position combines the activities of direct and indirect patient care and provides direct care to pediatric, adolescent, adult, and geriatric patients. The Staff Nurse directs CHHAs and CNAs in patient care tasks, performs patient assessments, develops patient care plans, evaluates the plan effectiveness, modifies it as appropriate and documents all patient care provided.

• Provides those services requiring substantial special skills within nurses scope of Practice Act.

• Initiates appropriate preventative and rehabilitative nursing procedures.
• Informs the physician and other personnel of changes in the client's condition and needs.
• Counsels and instructs the client and family in meeting nursing and related needs.
