Job summary

VitalCaring Group in Mississippi is seeking an experienced Home Health RN to lead patient care in patients' homes across an assigned territory.

You will perform in-home assessments, develop care plans, and coordinate with physicians and therapists to reduce hospitalizations and support independence.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience, home health or related acute care preferred.
• Strong clinical judgment and independent thinking.
• Excellent communication and patient-centered approach.
• Proficiency with EMR systems and documentation.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate status and goals.
• Develop and manage individualized care plans with physicians.
• Deliver skilled nursing care including meds and treatments.
• Serve as case manager coordinating across therapists and physicians.
• Monitor progress and adjust plan based on conditions.
• Educate patients and families for disease management.
• Coordinate transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation.

Role Overview

As a Home Health RN, you are the clinical lead for your patients - managing the plan of care, coordinating across disciplines, and driving outcomes from admission through discharge.

You’ll combine advanced assessment skills, care planning, and patient education to reduce hospitalizations, improve recovery, and support long-term independence at home.

What You’ll Do

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care
• Develop and manage individualized care plans in collaboration with physicians and care teams
• Deliver skilled nursing care, including medication management, treatments, and interventions
• Serve as the case manager, coordinating care across therapists, aides, and physicians
• Monitor patient progress and make clinical decisions based on changing conditionsEducate patients and families to support disease management and independence
• Coordinate care transitions and discharge planning to reduce readmissions
• Maintain timely, accurate documentation in the EMR
• Supervise and support Home Health Aides as needed
• Participate in on-call rotation as appropriate

What You Bring

• Active RN license (or compact eligibility) + CPR certification
• Graduate of an accredited nursing program
• Minimum one (1) year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred)
• Strong clinical judgment and ability to think independently
• Excellent communication skills and patient-centered approach
• Ability to manage a caseload and prioritize effectively
• Proficiency with EMR systems
• Valid driver’s license and reliable transportation
