This full-time role requires an active IL RN license, CPR certification, a valid driver's license, and at least one year of medical-surgical nursing experience.

Qualifications

In this role, you will manage a patient caseload, coordinate care plans, and collaborate closely with LPNs and an interdisciplinary team to deliver high-quality, patient-centered care. Schedule Full-time, Monday-Friday 8:00 AM - 4:30 PM Weekend rotation required On-call rotation required $3,000 Sign-On Bonus Available for External Candidates! Position Overview

The RN Case Manager is responsible for assessing, planning, coordinating, implementing, and evaluating care for patients receiving home health services. This role serves as the primary clinical coordinator for assigned patients and works closely with physicians, LPNs, therapists, social workers, and other healthcare professionals to ensure optimal outcomes.
