Job summary

PruittHealth Corporation is hiring Home Health Registered Nurses (RNs) to provide compassionate, one-on-one patient care in the home setting. The role emphasizes flexibility, strong clinical support, and opportunities for growth within a fast-growing RN specialty.

The Home Health RN Case Manager will conduct skilled visits, develop care plans, and coordinate with the care team, while mentoring LPNs and aides. A driver’s license and CPR are required.

Qualifications

• Active, unrestricted RN license.
• CPR certification.
• Valid driver’s license with reliable transportation.

Responsibilities

• Provide skilled nursing visits in the patient’s home.
• Complete assessments, start-of-care visits, and care plans.
• Collaborate with the interdisciplinary care team.
• Supervise LPNs and Home Health Aides.
• Document care using an iPad with AI assist (OASIS experience a plus).
• Educate patients and families to promote independence.
• Ensure continuity, quality, and safety of patient care.

What We Require

• Experience with aging or adult patient populations
• Highly organized with strong time-management skills; able to multitask, prioritize a busy caseload, and work confidently with technology and electronic documentation systems
