Job summary

Enhabit Home Health & Hospice in Warren, OH is seeking a licensed professional to provide skilled home-based care, following physician orders and the established plan of care. You will teach patients and families and collaborate with the health care team to ensure quality outcomes.

Responsibilities include administering care, coordinating with physicians, and maintaining dependable transportation while meeting state licensing and CPR requirements.

Qualifications

• Must have CPR certification for the healthcare provider.
• Must possess basic demonstrated technology skills, including operation of a mobile device.
• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience as a licensed professional.

• Administer skilled care to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).

Education And Experience, Preferred

• Experience as a licensed professional may be deferred with a review and approval from the regional vice president or regional director.


• Medicare home health or hospice experience is preferred.

• Must maintain automobile liability insurance as required by law


• Must maintain dependable transportation in good working condition


• Must be able to safely drive an automobile in all types of weather conditions


• Must possess CPR certification for the healthcare provider
